Background: The 1986 Cerritos mid-air collision stands as one of the most remarkable and pivotal disasters in civil aviation history due to the extraordinary convergence of human error, technological limitations, and catastrophic ground impact. On August 31, 1986, a private Piper PA-28 inadvertently strayed into the restricted Los Angeles Terminal Control Area without air traffic control clearance, placing it directly in the flight path of Aeroméxico Flight 498, a McDonnell Douglas DC-9 on approach to LAX. The small plane's propeller severed the airliner's horizontal stabilizer at 6,560 feet, sending the upside-down DC-9 plunging into a residential neighborhood in Cerritos, California, destroying five homes and claiming 82 lives—including 15 people on the ground. Beyond the tragedy itself, the accident was exceedingly rare in how profoundly it reshaped global airspace safety: it served as the direct catalyst for mandatory installation of Traffic Collision Avoidance Systems (TCAS) on all commercial aircraft and required Mode C altitude-reporting transponders for general aviation entering controlled airspace, virtually eliminating undetected mid-air collisions in major commercial flight corridors worldwide.
