Here is a clothing account form dated Sept. 14, 1864  Printed on both sides and in nice condition. This is a full ledger sheet headed at the top:

* The United States: In Account with Charles Wilson, private, 190th Regiment, Pennsylvania Volunteer on account of Clothing during his enlistment; the money value of each issue being hereby acknowledged.

The form is part printed & part handwritten. Included on the form are columns noting "Date of Issue" "Money Value" "Rank" "Signature" and "Witness", with a signature at the bottom.

The reverse of this form is similar, but for a James Williams, a private who enlisted on Sept. 17, 1864.
A nice Civil War item with documentation on both sides. Measures about 10 by 15 inches, a little irregular along the one margin, nice condition
